About Us

About Us

The Diabetes and Heart Disease Prevention and Control Program, housed within the Delaware Division of Public Health’s, Health Promotion and Disease Prevention Section, is committed to improving cardiovascular health and reducing the burden of chronic diseases across the state. Our efforts are aligned with the Centers for Disease Control and Prevention’s (CDC) National Cardiovascular Health Program (DP23-004), which provides funding and strategic direction to support states in advancing heart health through prevention, early detection, and management of cardiovascular conditions.

In Delaware, we lead the Delaware Cardiovascular Health Learning Collaborative—a statewide initiative designed to bring together health systems, community organizations, and public health partners to share data, identify disparities, and implement evidence-based interventions. This collaborative serves as a key strategy in our work under DP23-004, fostering cross-sector partnerships and aligning resources to promote health equity, improve hypertension control, and reduce cardiovascular events.

Through data-driven approaches, community engagement, and clinical-public health integration, we are building a sustainable model for cardiovascular health that addresses the unique needs of Delawareans, especially those in high-priority populations.

Workgroups

The Delaware Cardiovascular Health Learning Collaborative Workgroups bring together health care providers, community organizations, and public health leaders to improve cardiovascular health outcomes across the state. These workgroups serve as a platform for collaboration, sharing best practices, and developing innovative strategies to reduce heart disease and hypertension disparities.

Our work focuses on identifying challenges, implementing data-driven interventions, and promoting health equity in cardiovascular care. By working together, we aim to create sustainable solutions that empower individuals and communities to lead healthier lives.
